Your First Visit

The purpose of the first visit is to gather all pertinent information regarding your health to determine if yours is indeed a chiropractic case.  Your intake forms will be evaluated and you will receive a consultation, a complete examination and x-rays as needed.  You can download the intake forms in advance and bring them with you to help expedite the process (Link provided here).  If you bring your completed intake forms with you, you should set aside a full hour for this visit.  If you want to complete the forms in our office, please allow an additional 15 minutes.
