1. | The entrance to RCO at 316 East 59 th Street. |
|

|
1. | As you enter you will find yourself in the reception room where our friendly receptionist will greet you. Find the daily Sign In Sheet and sign in.
|

 |
|
|
|
|
|
|
|

|
2. |  On your first visit you will need to fill out our New Patient Forms (these can be downloaded and filled in prior to coming into the office). Please bring your insurance information with you. |

3. | The receptionist will then take you on a tour of the office so that you become familiar with it. |
|
 |
|
 |
|
1. |  Take your paperwork and gown and go to the changing room that is displayed on the clipboard.
 Turn on the light in the changing room. |

|
|||||
|
|
|
2. | When you have put on your gown, with the opening in the back, and are ready to see the doctor, switch on the amber light. |
|
 |
|
|
|
|
|||||
|
|
|
3. | The doctor will open the door and greet you once he is ready to see you. |
